All by Myself is the tenth episode of the fifth season of Grey's Anatomy, and the eighty-eighth episode overall.
Cristina earns her first solo surgery among residents but has to relinquish it and choose her replacement, while Mark's cutting-edge procedure could help restore a patient's speech. Also, the doctors deal with quarreling sisters who were injured in a car accident involving their parent's car.

Plot Overview
One of the side plots involves sisters Holly and Emma Anderson, who are always quarreling and bickering. Holly was irritated that she had to pick up her sister, and Emma was upset that Holly was texting while driving their parents' car. The texting causes an accident and both sisters are brought into the hospital for treatment of minor injuries, or what appears to be anyhow.
In the emergency room, the two sisters continue arguing and blaming each other. Emma, who had designs on going to a Future Leaders Of America conference in Washington, D.C., wants to call her parents, but Holly is really angry because she knows she'll be punished. It is clear they do not get along at all, even to the point where they wished each other would go away ... forever.
That moment actually does happen – wanting the other to go away, forever – when Alex does a routine eye check and notices something very wrong ... it may be a sign of a basilar skull fracture, and that it needs immediate attention. Emma sees the medics wheeling Holly out of the room and is relieved. "Good! I hope you die!" she screeches.
In the emergency bay, Derek and Alex look over the brain scans and order a cardio consult for Holly, but when she goes into shock and begins bleeding from the nose and brain matter is in the blood, they order emergency surgery. Emma realizes what is going on and screams Holly's name, now seeing that her sister's life is in danger.
Emma constantly asks about Holly and whether she'll be alright, but eventually she'll have to be told. Despite the doctor's best efforts, Holly had a massive stroke following the accident and while some repairs were made, she would likely live the rest of her life in a semi-vegetative state. The Andersons' parents arrive and, after being told that Holly is brain dead, they eventually with some persuasion consent to donating her organs. Shortly before Holly is taken off life support, Emma goes into her room and completely breaks down at her bedside, telling her she loves her more than anything and she was sorry for what she said earlier and all their fighting and their feuding, and that she'll miss her more than anything.
