In this episode, Gordon from season two, Bloodlust comes back. If you remember he was a dangerous vampire hunter who in Bloodlust was tied up by Dean in the end and left there.

Instead of the usual outline of the two brothers hunting something supernatural and killing it, they are the ones being hunted by Gordon who thinks that Sam is going to do something evil in the future.

Sam meets another psychic of the name Ava, who has the same type of powers as Sam. She finds Sam (who is away from Dean at the moment in order to "find himself") and warns him of a vision she had with Sam ending up dead via a grenade and trip-wire.
