Desperate to get his brother out of solitary confinement, Michael threatens the warden in order to carry out his escape plan. As Michael and the escape team executes the plan, their success in gaining entry into the infirmary now rests on Dr. Sara Tancredi. Sara must make a decision whether she should help Michael or not. Veronica's plan to Blackfoot, Montana is hindered by her closest ally, who has a secret that could endanger her life. The escape team makes a break for it, but not without raising an alarm.
