Season Nine of Scrubs premiered on December 1, 2009. The series drastically shifted this season due to major cast changes. The hospital has been relocated onto the campus of a medical school and only two of the original seven principle actors continue their roles as regulars throughout the season: Donald Faison and John C. McGinley. Zach Braff is credited as a regular, although he only appears in the first six episodes of the season. Early reports suggested that Neil Flynn would also resume his role until the pilot for The Middle was picked up by ABC, in which he stars opposite Patricia Heaton. Reportedly, show creator Bill Lawrence wanted to change the show's name this season, and although it is officially still known as Scrubs, the title card now features 'Med School' as a subtitle of sorts.

Faison and McGinley are joined by "a quartet of newbies (most of them playing students)" as full-time regulars, including Eliza Coupe, who recurred as Denise "Jo" Mahoney last season. Also among the new principles are Dave Franco, brother of James Franco, and Kerry Bishé, who takes over narration duties from Braff. All of the remaining original actors made guest appearances on the show, with the exception of Judy Reyes.
